Animal Communication Session form is now electronic!  

I am migrating to electronic forms that make it simpler to complete, sign, and return... and save paper!  

You will receive an e-form to completed and e-sign after you have completed scheduling your session.   Pet photo can be emailed

Animal Reiki Session -(electronic coming soon, but until then forms are below, that need to submitted)

You must complete and return both the Animal Reiki Client Information Form and the Animal Reiki Client Waiver Form prior to the scheduled Reiki Session.

The completed form(s) can be attached and submitted at the bottom of this page, or emailed to me at feelthemspeak@gmail.com .  Thanks!
